Before Your First Visit
• You may Download, complete, and bring the New Client Packet here, or complete and submit electronically by clicking the link below.
• Bring a list of all medications including daily dosages
• Bring insurance information
• Bring drivers license or identification card
• Expect your initial visit to last approx. 50-60 minutes

Please note: If you are unable to keep your scheduled appointment, please contact within 24 hours prior to your appointment time.
Distance Counseling Clients

• Please Complete "Distance Client Packet"
• Attach insurance information if applicable*
• Attach a copy of your photo identification
• List all medications including daily dosages
